  As legal guardian of the registered player, I herby consent to the aforementioned person participating in the Texas Express Basketball Club programs, tryouts, practices, and/or games. I recognize that severe injuries can occur in any activity that is associated with basketball, and understand that it is the express interest of the Texas Express Basketball Club to provide for the safety and protection of my child. In consideration of allowing my child to play for the Texas Express Basketball Club, I hereby forever release the Texas Express Basketball Club, First Baptist Church of Grapevine, Game on Sports Complex Fort Worth, Lewisville MAC, Fieldhouse USA Frisco, Mansfield Fieldhouse USA, Grapevine Fieldhouse USA and/or any other facility used by Texas Express Basketball Club, it's officers, employees, coaches and owners from all liability for any Covid 19 spread and all damages and injuries suffered by my child while under the instruction, supervision, or control of an of the above so mentioned. As legal guardian of the aforementioned person, I hereby agree to individually provide for the possible future medical expenses which may be incurred as a result of any Covid-19 spread or injury to my child sustained while training at, practicing at, competing at, and/or trying out for the Texas Express Basketball Club and/or any of the formentioned facilities or any other facility not listed which could be used by the Texas Express Basketball Club. In case of emergency, I authorize the Texas Express Basketball Club staff to administer first aid to my child and/or take my child to a physician or hospital for further treatment. This acknowledgement of risk and waiver of liability, having been read thoroughly and understood completely, is signed (by checking the box below) voluntarily as to its content and intent.
